Parental Rights and Responsibilities
License Cancellation
A parent/guardian who signed the Financial Responsibility Statement may cancel a minor’s license at any time before the driver turns 18.
To Cancel:
- Complete the Minor Affidavit (DP-38)
- • Bring the form and the license to a DMV office
- • The license must be surrendered

- Parents/guardians who knowingly allow an unlicensed or non-compliant minor to drive may be held legally responsible for:
- Fines
- Restitution after a citation or accident
In-Person Reinstatement
Any adult who serves, sells, or allows alcohol or controlled substances to be used by a minor on their property may face civil liability for any resulting damages.
