State of Record
NV for drivers licensed in Nevada.

Medical Certificate Expiry, Status & Self-Certification
Some commercial drivers must pass a physical exam every two years and keep a current medical certificate. They must also certify the type of driving they do.

Mailing and physical address on file; physical address appears on license.

Licn Type, Class & Restrictions
Lists type of ID (ID Card, Non-Commercial DL, Commercial DL). Also includes license classes/endorsement (A, B, C, M) and any restrictions. References NAC Chapter 483 and Endorsements/Restrictions.
License and ID Card Types Table
ID Card: Nevada ID Card; NCDL: Non-Commercial DL; CDL: Commercial DL; Class A: 26,001+ lbs, trailer 10,001+ lbs; Class B: 26,001+ lbs, trailer ≤10,000 lbs; Class C: ≤26,000 lbs; Endorsement M: Motorcycle.
Status (ID/License/Permit)
Indicates if ID, license, or permit is canceled, expired, limited-term, revoked, suspended, or valid.
Issue Date
Date latest valid license/card was issued. Not original issue date. Proof of first Nevada license requires IR 002.
Expiration Date
For individuals 70 years old and under, the expiration date is generally their birthday. For individuals 71 and older, the expiration date is generally their birthday plus four years.
Endorsements

Convictions & Violations

Conviction Listings
Moving violation convictions reported by courts. Dismissed or reduced offenses are excluded.
Demerit Points
Number of active demerit points. Points expire after 12 months from date of conviction. 12+ points = suspension.
Cite Date & Conv Date
Citation date (issued by law enforcement) and conviction date (by court).
State & Court
State and court code for conviction. Nevada courts use DMV codes; out-of-state = 999.
Viol Code & Off Ty (Violation & Offense Type)
Violation code and offense type per DMV Code Search. Offense type also reported to PDPS and CLDIS for CDL.
CMV Off & Haz Mat
Shows whether offense was committed in a Commercial Motor Vehicle (CMV) and if hazardous materials were involved.
Citation Number
Case number on file with law enforcement and court. Matches withdrawal listing if applicable.
Any convictions older than 3 years will be on the 10 year driver history report.
